Pharmacist License

Note: The address has been removed from the certificate, however, the Department of Consumer Protection must be notified of any name, address or employment change within five (5) days of such change.  Changes and questions can be emailed to the Drug Control Division at DCP.PharmacyCommission@ct.gov.
In the State of Connecticut, a Pharmacist can obtain a license by four different means:

      All licenses expire biennially on January 31st of every even-numbered year. Fee: $120
