Note: The address has been removed from the certificate, however, the Department of Consumer Protection must be notified of any name, address or employment change within five (5) days of such change.  Please log in to your account on www.elicense.ct.gov and go to license maintenance to update your information.  Questions can be emailed to the Drug Control Division at DCP.PharmacyCommission@ct.gov.
 
In the State of Connecticut, a Pharmacist can obtain a license by four different means:
