COVID-19 vaccinations administered in Connecticut are reported into CT WiZ providing you with a complete, consolidate record. CT WiZ COVID-19 vaccination records are SMART Health Cards. These are currently available by mail or fax upon request or through the CT WiZ Public Portal.

SMART Health Cards allow you to store your vaccination history securely on your smartphone or onto an app, or you can simply save or print it for future use. The QR code can then be scanned by a compatible SMART Health Card verifier app at places like airports or large venues that require proof of vaccination, if you choose to share it.
