Preface

The Transition Assessment Resource Manual was originally developed in 2008 by the Connecticut State Department of Education (CSDE), in collaboration with the State Education Resource Center (SERC) and the Connecticut Transition Task Force (TTF), in response to requests from educators, administrators from local education agencies (LEAs), and families seeking guidance on age-appropriate transition assessments. In 2023, the CSDE, in partnership with the Connecticut Alliance of Regional Educational Service Centers (RESC Alliance), SERC, and the TTF, revised the manual to update existing assessments and expand the range of assessment options across each transition area. The most recent update occurred in 2025, further refining assessment listings and ensuring continued relevance to current practices. While every effort has been made to keep the manual as current as possible, readers should be aware that published assessments are frequently revised. As such, new assessments may have become available since publication. Neither the CSDE, the RESC Alliance, nor SERC endorse any specific assessment included in this manual.
