Preface

The Transition Assessment Resource Manual was created in response to educators and administrators from local education agencies (LEAs) and families seeking information about age-appropriate Transition Assessments. It was prepared in 2008 by the Connecticut State Department of Education (CSDE) in collaboration with the State Education Resource Center (SERC) and the Connecticut Transition Task Force (TTF). In 2023, the CSDE, in collaboration with the Connecticut Alliance of Regional Educational Service Centers (RESC Alliance), the SERC, and the TTF, revised the manual in order to update current versions of assessments, and expand assessment options for each transition area. Although every effort was made to ensure that the current manual is as up to date as possible, published assessments are revised on a regular basis. Therefore, readers of this manual should be mindful of possible new additions of assessments. Neither the CSDE, the RESC Alliance, nor the SERC endorse any particular assessment listed in this manual.
