Ntact:C Assessment Guide and Supplements for Transition Planning
The National Technical Assistance Center on Transition: the Collaborative (NTACT:C) has developed The Collaborative Assessment Guide for Transition Planning, an updated and expanded version of their Age Appropriate Transition Assessment Toolkit.
This guide is intended to help develop a coordinated assessment approach for transition planning and service delivery. The guide is designed to help include students, family members, educators, vocational rehabilitation counselors, human services and health agency staff, and many others.
The assessment process consists of six separate, but interconnected processes, and the guide is organized by them. They are:
- Section 1: Determining What to Assess,
- Section 2: Determining Stakeholders,
- Section 3: Selecting Appropriate Assessments,
- Section 4: Conducting Assessments,
- Section 5: Reviewing Assessment Results, and
- Section 6: Using Assessment Data.
The guide also includes five related supplements:
- Collaborative Assessment Guide for Families (which includes an informal questionnaire for families),
- Collaborative Assessment Guide for Students (which includes several worksheets for students),
- Collaborative Assessment Guide Regulations and Procedures (which highlights the assessment requirements of the IDEA and the Rehabilitation Act of 1973, as amended by title IV of the Workforce Innovation and Opportunity Act),
- Tools and Resources (which includes a few resources to assessment tools (formal and informal) across domains which may be considered when collecting, analyzing, and using data to support transition planning with students), and
- Collaborative Assessment Guide Definitions (which includes a list of definitions related to assessment that are used throughout the NTACT:C guide and supplements).
