Accepting/Rejecting Infant Formula Form
The CACFP facility must offer at least one type of iron-fortified infant formula. A parent or guardian may choose to accept the offered formula, or decline the offered formula and supply expressed breast milk or an iron-fortified infant formula instead.

CACFP facilities must keep daily menu records of all meals and snacks served to each infant enrolled in the CACFP.
