Connecticut Integrated Care for Kids
Overview
The Department of Social Services administers a Targeted Case Management (TCM) program in New Haven that provides coordinated, child-centered support to eligible HUSKY Health members through Connecticut Medical Assistance Program (CMAP) local community service providers, known as Connecticut Integrated Care for Kids (CT InCK). The program is designed to improve care for children under age twenty-one (21), as well as pregnant and postpartum individuals covered by Medicaid or CHIP, by promoting prevention, early identification, and treatment of priority health concerns.
Local community-based service providers play a central role in delivering TCM services to individuals who may qualify if they are under 21 years of age or are currently pregnant or within one year postpartum and reside in a New Haven ZIP code.
CT InCK providers conduct needs assessments, determine the individual’s Service Integration Level (SIL), and coordinate the recommended level of care. They work directly with eligible members to identify barriers, arrange appropriate medical, behavioral, and social services, and ensure consistent communication among healthcare professionals, caregivers, and support networks. Their involvement ensures that each individual receives timely, coordinated, and comprehensive support tailored to their specific needs.
