The Connecticut Medical Home Family Satisfaction Survey is a way for families to give feedback to their providers and to the State Department of Public Health about the care that they and their child with special health care needs have experienced.
It gives families a chance to comment on three parts of the Medical Home system - the primary care provider, the care plan, and the care coordinator - asking specific questions about each one that help to point out areas of strength and weakness.
The survey is anonymous and confidential which means that you do not have to worry about anyone linking your name to your answers.
You only need to take this survey once. If you have already taken this survey, you do not need to take it again.
