Pharmacist License Reinstatement
All reinstatement applicants must provide copies of continuing education (CE) certificates for the two years immediately preceding the date of the reinstatement application.
Pharmacists are required to complete fifteen (15) credits of continuing education per calendar year (January 1 - December 31).
- 1 credit or contact hour = 0.1 CEUs
- 5 credits of the 15 credits must be obtained at a live presentation
- 1 credit of the 15 credits must be on the subject matter of pharmacy law or drug law

License Inactive 2 - 5 Years
CT Pharmacist licenses that have lapsed for two or more years, but less than five years, must submit the following:
- Online application and fee. The fee is based on the date the license lapsed.
- Continuing education. Verification of successful completion of continuing education for the two-year period immediately prior to the application for reinstatement.
License Inactive 5 Years or More
Pharmacists Licensed by Reciprocity Who are Applying for Reinstatement
Pharmacists licensed by reciprocity to practice pharmacy in the state of Connecticut and who have allowed their license to lapse for five years or more must complete the license process by reciprocity again.
Pharmacists Licensed by Examination Who are Applying for Reinstatement
Pharmacists licensed by examination to practice pharmacy in the state of Connecticut and who have allowed their license to lapse for five years or more must submit:
- Online application and $200 reinstatement fee.
- Verification of successful completion of continuing education for the two-year period immediately prior to the application for reinstatement.
- Verification of successful completion of the Multi-State Pharmacy Jurisprudence Examination (MPJE).
- Contact the National Association of Boards of Pharmacy (NABP) for examination registration and fee information.
How to Apply for Reinstatement Online
- Go to https://www.elicense.ct.gov
- Log in to your account using the User ID and Password used for your pharmacist license
- You will need to know your User ID, Password and License Number
- If you don't know your User ID or Password, click on "Forgot Password?" or "Forgot User ID?"
- If you do not have your license number, send an email to DCP.PharmacistLicense@ct.gov
- For Category, select "Drug Control"
- Click "Start" next to Pharmacist (PCT) to start your reinstatement application
