Register for a Medical Marijuana Card

Qualifying patients who register with the Medical Marijuana Program enjoy these benefits:

  • Ability to purchase larger quantities (up to 5 ounces per month)
  • Higher personal possession limits (up to 5 ounces vs. 1.5 ounces for individuals who are not registered as qualifying patients)
  • Medical marijuana and adult-use cannabis products purchased by a qualifying patient or caregiver are tax-free
  • Income-based financial assistance programs at dispensary facilities and hybrid retailers
  • Access to products not allowed in the adult-use cannabis market, such as pills, tables, and sublinguals
  • No potency caps on flower or other cannabis products
Qualifications

To qualify for a medical marijuana card:

  • You must be a resident of Connecticut;
  • You cannot be an inmate in a correctional institution or facility under the supervision of the Department of Corrections; and,
  • You have been certified by a Connecticut-licensed physician, advanced practice registered nurse (APRN) or physician assistant (PA) who is treating you for a qualifying debilitating medical condition.
Before You Start the Application

Only your physician, physician assistant or advanced practice registered nurse can start your application by certifying that you have a medical condition that qualifies you to register as a medical marijuana patient.

The first step you must take is to make an appointment with your physician, physician assistant or advanced practice registered nurse.

If your provider decides to certify you for a medical marijuana registration certificate, you will need to provide them your:

  • Valid email address: This will be the primary way the Department will contact you about your medical marijuana registration. Make sure this is an email address you can access and are comfortable using for your medical marijuana registration.
  • Primary telephone number: This should be a personal telephone number that the Department can use to contact you about your medical marijuana registration.
Documentation Requirements

To register as a medical marijuana patient or caregiver, you must fill out the online application. Before you start your application, you should have these documents saved on your computer or device, ready to be uploaded into the system:

  • Proof of Identity: Submit a copy of a form of identification that is not expired. Acceptable forms include:
    • Connecticut or Out-of-State Issued Driver’s License
    • Connecticut Issued ID
    • Connecticut pistol or firearm permit
    • US Passport or Passport Card
    • Permanent Resident Card
    • Certificate of Naturalization
    • Certificate of Citizenship (some age restrictions apply)
  • Proof of Connecticut Residency: You must submit one document to prove that your home is located in Connecticut. The document must:
    • Show your name and your Connecticut residence address;
    • Be dated within 90 days (unless stated otherwise below); and
    • Be computer generated (not typed).

Acceptable documents include:

  • Computer-generated bill or statement from a bank or mortgage company, utility company, doctor or hospital
  • Pre-printed pay stub showing both your name and address and your employer’s name and address
  • W-2 form property or excise tax bill, or Social Security Administration or other pension or retirement annual benefits summary statement and dated within the current or prior year
  • Medicaid or Medicare benefit statement
  • Current valid homeowner’s, renter’s or motor vehicle insurance policy dated within the last year
  • Current motor vehicle loan statement for a motor vehicle registered in your name
  • Residential mortgage or similar loan contract, lease or rental contract showing signatures from all parties needed to execute the agreement and dated within the last year
  • First-class mail addressed to your home address
  • Connecticut voter registration card
  • Survey of your Connecticut property issued by a licensed surveyor
  • Connecticut handgun permit
  • Motor vehicle registration
Applying Online

You are ready to begin filling out your application once:

  1. Your provider has started your application and certified you;
  2.  You have your Proof of Identity saved on your computer or device; and,
  3. You have your Proof of Connecticut Residency saved on your computer or device

To fill out the application:

  1. Go to https://biznet.ct.gov/dcp-mmrp. Create an account on BizNet, the Department of Administrative Services’ Business Network.
  2. Verify your email. Once you have created an account, check your email to verify your account.
  3. Log in to https://biznet.ct.gov/dcp-mmrp. Use the email address and password you entered when creating your account.
  4. Select “I Am a Patient”. Enter in your date of birth and email address to verify you are the correct patient. Review the information on the Patient Information page and make sure it is correct.
  5. If your provider authorized a caregiver based on your debilitating medical condition, provide your caregiver’s information, including their email address.
  6. Upload your documents. Upload the Proof Identity and Proof of Connecticut Residency documents from your computer or device into the online system.
  7. Answer the attestation questions. These questions confirm the patient's understanding of the responsibilities and requirements of participating in the program.

Once you have completed the application, the Department will review the information submitted and make a decision to issue or not issue you a medical marijuana registration certificate. Your certificate expires one year after you were certified by your provider. You must renew your certification every year to participate in the Medical Marijuana Program.
