RE:	Acknowledgments of Physical Condition

Public Act 95-277 dealing with the Second Injury Fund has repealed Section 31-325, the Acknowledgment of Physical Condition. Consequently, effective June 29, 1995 the Commission will no longer be receiving and filing Acknowledgment of Physical Condition forms.
