Individuals who wish to apply to take a certification examination are to complete the appropriate application form and submit it with the non-refundable $224 application, via mail delivery, to the below address:

Prior to formal submission of the application with the non-refundable application fee, applicants may wish to submit their completed application via email to dph.opcert@ct.gov to request an informal review.

Applicants will be informed of your confirmation or rejection for the examination via email from this office. If confirmed, applicants will receive an additional email from the test center administrator, PSI, to schedule your computer-based examination. Confirmed applicants will be required to pay a $102.00 examination fee to PSI. Mastery report results will be available to you upon completion of the examination and also provided to the CT DPH, Operator Certification Program.
