HIPAA Notice

The Department of Public Health (the “Department”) is a “hybrid entity” as defined by the

Health Insurance Portability and Accountability Act of 1996, as amended (“HIPAA”).

The Department’s laboratory and its support services are the only areas of the Department that

perform business activities that are covered functions under HIPAA. The following notice of privacy
