This self-blood pressure monitoring (SBPM) toolbox is the result of a pilot project that established a SBPM program at two community health clinics in Connecticut.
 
This team-based care intervention provides support and close follow-up by specially trained Nurse Care Coordinators who work with patients individually to teach them how to monitor their own blood pressure, set self-management goals and develop action plans. They assist patients to attain blood pressure control more rapidly and effectively than they otherwise would have. 
 

The toolbox contents are specifically designed to allow for easy replication and adaptation of a SBPM program to other clinical sites. It includes an assortment of templates and materials needed for replication of a SBPM program.

  1. Universal Policy – overview of the program protocol outlining enrollment procedures, roles and responsibilities of clinical members, implementation, and follow-up
  2. Universal Playbook – details a team-based approach using Panel Management sessions and Care Coordination; lays out, step-by-step, each team member’s role in the process
  3. Universal Excel Template – used for data collection and tracking; may be linked with EMR to auto-populate fields and prevent duplication of data entry
  4. SBPM Pilot Project Video – educational/informational video highlighting the SBPM Pilot Project including the overall process, patient and provider benefits, and program replication.
  5. Sample Patient Education Materials – including some created specifically for the program, used to track labs, medications, self-management goals, blood pressure readings, etc.
