To report your MOR and/or NAR electronically, please fill out the Subscriber agreement below and submit it to: DEEP.WPCF@ct.gov.  If there are changes to the authorized reports, you must fill out Part D and E and submit the entire for to DEEP.WPCF@ct.gov
