Example Annual Transition Goal
Given (insert condition), Student will plan, attend, participate in and/or facilitate their Planning and Placement Team (PPT) meeting, as measured by increase from baseline of (insert current score) to (insert target score) on PPT participation rubric by (insert Date).
Example Short-term Objectives
- Given a PPT agenda template, Student will build the agenda and create a plan to participate and/or facilitate, as measured by increase from baseline of (insert current score) to (insert target score) on PPT participation rubric by (insert Date).
- During the PPT meeting, Student will collaborate with the PPT members to finalize the secondary transition planning section of the IEP as measured by increase from baseline of (insert current score) to (insert target score) on PPT participation rubric by (insert Date).
- Given access to technology, Student will use digital media and/or visual displays during the PPT meeting to share a plan for post-school life as measured by increase from baseline of (insert current score) to (insert target score) on rubric outlining contents of presentation as well as PPT participation rubric by (insert Date).
Additional Transition Activities/ Services to Consider
- Skill training to develop a neat appearance (hair, proper use of make-up, clothing).
- Prepare for PPT meeting by creating an agenda, slide show, etc.
- Practice facilitating their PPT meeting.
- Student writes a journal entry about a challenge they possess and how it can be utilized as a strength. In their next journal entry, student discusses a strength they possess and how they can use it to their advantage.
