Do I need to submit a CON form to replace our MRI, CT, PET or PET-CT.
To change a scanner you already received permission to have:
- Go to the CON Forms and Fees page
- Download and fill out the CON Replacement form
- Upload your form to the Certificate of Need Portal
We will review your form and make sure you had previous permission. Then, we will post a notice on the Certificate of Need Portal and notify you.
