Understanding the COREQ Survey for Nursing Homes
What is the COREQ Survey?
The CoreQ Survey (COREQ) is an annual resident and family satisfaction survey conducted in all Medicaid-certified nursing homes in Connecticut. Its purpose is to provide transparent, standardized feedback about each nursing home’s performance from the perspectives of residents and their families.
How is COREQ Used?
- Residents and families can use the survey results to compare nursing homes and help decide where to receive care.
- The survey results offer real-world insights into the experiences of people living in or visiting each nursing home, such as care quality, responsiveness, environment, and staff interaction.
- Residents and families can use the information to ask questions or request improvements through the resident/family council or facility management.
How are the Results Collected?
- Each year, an independent organization (UConn Center on Aging) surveys residents and family members from every Medicaid-certified nursing facility in Connecticut.
- The Department of Social Services (DSS) publishes the results and provides summary reports for public review.
- 2025 results can be downloaded:

Why is COREQ Important for Residents and Families?
- You can see how residents and families rate their satisfaction with each facility, which helps guide decisions and set expectations.
- The results allow for comparisons between different nursing homes using consistent, unbiased measures.
- Residents and families can use the data to raise concerns or suggest improvements to the resident council or facility management.
How to Use COREQ Results
- Discuss the results during resident/family council meetings.
- Use the data to raise any concerns or make suggestions for improvements.
- Support requests for changes to care, environment, or procedures at the facility.
- When selecting a nursing home, review recent scores to inform your decision.
