Welcome to the provider website for HUSKY Primary Care, Connecticut’s Primary Care Case Management (PCCM) program. The purpose of this site is to share information and resources with participating Primary Care Providers (PCPs) and other Provider Advisory Group members.

enroll directly with participating Primary Care Providers instead of enrolling in a HUSKY managed care health plan. |
Participating PCPs are responsible for coordinating members’ healthcare. In turn, they receive a monthly care coordination / case management fee of $7.50 for each member, in addition to fees for services provided. Medicaid programs in several other states use PCCM successfully to improve patients’ clinical outcomes, and both client and provider satisfaction, while at the same time controlling health care costs.
The Pilot Program began in February 2009 in the greater Waterbury and Windham areas. For enrollments beginning January 2010, it is available in the greater Hartford and New Haven areas.
