ADA Self Evaluation

The Department of Revenue Services is conducting an ADA self evaluation to ensure that our programs, services and activities when viewed in their entirety are accessible to people with disabilities.  You are invited to participate by submitting comments on the fillable self evaluation questionnaire regarding the accessibility of agency programs, services, and activities to Penny Potter, EEO manager.  The form can be submitted three ways

  1. Electronically by email by clicking the submit button on the form
  2. Faxed to the fax number on the form, or
  3. Mailed to the address on the form.
