Guidance and Forms
All subawards require report and submission on the progress and status of awarded projects. Additionally, subawardees are responsible for completing and submitting timely requests for payment and other required federal and state forms.
DEMHS has moved to an electronic grants management system for the administration and management for federal nonprofit security grant program (NSGP-S) awards for FY 2024 and beyond. The following forms and reports either should be completed and uploaded within the EM Grants System, or should be sent to the DEMHS Program Manager at DEMHS.NSGP-S@ct.gov for any awards before FY 2024.

DEMHS NSGP-S Forms and Reporting Documents for Awardees
- Environmental and Historic Preservation Form - required to be submitted by ALL awarded sub-applicants. Approval must be received before projects can start.
- Procurement Standards
- Conflict of Interest Form
- Quarterly Progress Report - must be submitted within 30 days of the close of the quarter (March 31; June 30; September 30; December 31)
- Quarterly Financial Report - must be submitted within 30 days of the close of the quarter (March 31; June 30; September 30; December 31)
- Reimbursement Request Form - must be submitted with copies of associated invoices and proofs of payments
- Equipment / Property Inventory Report - must be submitted with the final reimbursement request
All forms and documents should be submitted to DEMHS.NSGP-S@ct.gov and/or uploaded to the EM Grants System. Please include your organization's official name, grant program, and award year and number on all correspondence.
