Well Drilling Completion Reporting

Well Completion Reports may now be submitted online at: https://elicense.ct.gov . Login with your user ID and password. If you don't recall them, please use the "forgot user ID" and/or "forgot password" links. If you are unable to retrieve your login information, please email dcp.online@ct.gov with your name and license number.

After logging in, select "File Well Drilling Report" under "Account " from "Online Services".

Select "Start" next to your well drilling license that applies.

Follow the prompts and answer the required questions, including uploading copies of the drawn map and approved permit. At the end, review and submit your report.

Select the type of report to be filed

This report will require entry of certain pieces of information and documents, including:

  • GPS coordinates of each boring location, to 5 decimal places (e.g. -71.12345 and 42.12345)
  • Uploaded image of the location map - can be a digital drawing or photo/scan of a hand drawing
  • Uploaded copy of your approved drilling permit - can be a legible photo or scan of the document
  • Optional: photos of the site/location

You will receive an email copy of your report at the address associated with your online account. You must submit this report to your local health department that approved your permit, as well as to the homeowner.
