To be a Wholesaler of Drugs, Medical Devices and/or Cosmetics, including controlled substances, you need to accurately complete the application on our website and submit it with the appropriate fee. Location(s) in Connecticut are subject to an inspection prior to being approved to distribute drugs, medical devices and or cosmetics from your location(s). If you distribute controlled substances, you must also provide a copy of your Drug Enforcement Administration (DEA) registration. For out-of-state wholesalers, in addition to your application, you must provide a copy of your home state license.
