W-1 Water Supply Driller Contractor

If you have held the W-2 for a minimum of two years, you must submit a copy of your W-2 license with the application.

If you are applying with equivalent out of state license, please note that an equivalent out of state license is one that requires 10,000 on the job training hours and 288 classroom hours, equivalent classroom curriculum, to obtain.  An equivalent license must be one that is individually held, rather than a business license or registration. Only one out of state license will be reviewed and considered for equivalency.

If you hold an out of state contractor license that meets this criterion, submit the following documentation: 

  • Copy of the license
  • Official license verification from the state in which you hold the license.
  • Laws and regulations that include the on the job training and classroom hours met for you to obtain this license and scope of work for this license.
    Documentation submitted for proof of equivalent an out of state license MUST BE in PDF format, scanned and attached to an email.  Screenshots from a mobile device are not accepted. You must clearly mark (i.e. underline highlight, circle) the area within the document that states the specific criteria and number of on the job training and classroom hours that were required to obtain this license.

If you are applying with equivalent experience and training, you must submit the following documentation:

  • Proof of on the job training -10,000 hours

Employer letters are required and must include dates of employment as well as a specific, detailed description of work experiences including all aspects of the scope of work.  Letters must be on company letterhead, signed, dated, and notarized. 

  • Proof of related instruction – 288 hours

Transcripts, diplomas, degrees and/or certificates are required.
