1. Gather your information
To make sure your complaint is processed as quickly as possible, gather all your details before you begin. You will need your basic contact information and your official Department of Corrections (DOC) number if you are currently in a jail, prison, or other correctional facility. If you are a family member or advocate filing on behalf of an incarcerated person, you do not need a DOC number.
Make sure you know the exact name of the correctional facility and the specific housing unit involved. Finally, write a list of any witnesses who can verify your complaint. This includes any other incarcerated individuals or facility staff members who saw what happened or have helpful information about the incident.

2. Fill out the form
Complete the required sections on the document
You will need to describe exactly what happened, where it took place, and what facility rules or state policies you believe were broken. Be sure to check "yes" or "no" if this issue has already been part of a court hearing or a prison grievance. If you have already filed a grievance with the facility, explain what their response was.
Remember, you must fill out a separate form for each individual complaint you wish to file with our office.

Online submission for faster processing
If you have access to a computer or smartphone, digital submission is your best option. First download the fillable PDF form directly to your device. Then, type open the form and type your answers into the boxes. Save you finished document. Then, open your email, and attach the completed form to a new message. Send your email directly to Correction.Ombuds@ct.gov.

3b. If you choose to mail your form
Physical submission by regular mail
If you do not have email access or prefer to use paper, you can submit your paperwork by mailing it. First, download and print out a blank copy of the form. Write your answers clearly, and feel free to attach extra sheets of notebook paper if you need more space to finish your story.
