Forms and Reports for Construction Contractors
- Good Faith Efforts Short Form Plan:
This plan is for use by contractors working on a public works project whose state-funding is between $150,000 but less than $1,000,000. This applies whether the awarding authority is a state agency, a quasi-public agency, a municipality, or a grantee of state-funding. - Good Faith Efforts Long Form Plan:
This plan is for use by contractors working on a public works project whose state-funding is $1,000,000 or more or by Construction Managers on projects with more than $150,000 in state-funding. - Bid Tabulation Worksheet:

This is a letter template to be provided to the contractor awarded a state-funded public works contract subject to the contract compliance requirements.
- CHRO - Street Paving Limited Exemption
For use by municipalities awarding a street paving contract. This exemption form will excuse the awarded contractor from having to file a Good Faith Efforts Plan should the contract meet the requirements of the exemption.
