Prevention Service Initiative Awards for Community-Based Organizations RFP - Due by October 10, 2018
Through a previous procurement, the State of Connecticut State Innovation Model (SIM) selected seven Community Based Organizations (including local health departments) and seven ANs/FQHCs (referred to as health care organizations) to work with the State to demonstrate the use of a CBO linkage model as part of Connecticut’s broader SIM strategy to promote healthier people, better care, smarter spending, and health equity. The initiative is limited to Bridgeport, New Haven, Middletown and surrounding towns. The goals of the initiative are to: 1. Increase the number of individuals with unmet prevention needs who complete community- placed, evidence-based prevention services and maintain or improve wellness. 2. Improve healthcare organizations’ performance on quality measures related to asthma or diabetes and associated ED utilization or admissions/readmissions for an attributed population through use of community-placed, evidence-based prevention services. 3. Enhance business competency skills and organizational capabilities of CBOs so they can enter into at least one contractual relationship with a health care provider that is participating in value-based payment. The Office of Health Strategy (OHS) is seeking one or more contractors to provide OHS with expertise in meeting facilitation, stakeholder engagement, health economics, actuarial modeling, and analytics services to develop Connecticut’s first health care cost growth benchmarks, primary care spending targets and quality benchmarks.OHS has extended the application due date from February 21, 2020 to March 2, 2020 at noon. Consumer Engagement Coordinator RFP - Due by January 14, 2019
The Office of Health Strategy (OHS) is seeking a Consumer Engagement Coordinator (CEC) to support and serve as the primary resource to its Consumer Advisory Board (CAB). Consumer and community input and engagement is critical to designing and implementing changes in the health system that benefit the Connecticut population. The CAB was created to ensure that the voice of the consumer is heard in all of the work of OHS. The CAB is particularly interested in strengthening the positive impact of innovations in health and health care on consumers, particularly those who are at-risk and underserved, strengthening the communication between OHS and consumers of health services and ensuring ongoing statewide engagement and input of consumer into the activities of OHS.
