HUSKY Primary Care - Case Management

HUSKY Primary Care, Connecticut’s Primary Care Case Management (PCCM) program, is a pilot program currently open to HUSKY A members who live in the Hartford, New Haven, Waterbury, and Windham areas. We will soon be expanding the program to the Putnam and Torrington areas.
 
With HUSKY Primary Care, you and your family members enroll directly with a participating Primary Care Provider, instead of enrolling first with a HUSKY managed health care plan and then selecting a Primary Care Provider. In HUSKY Primary Care, your Primary Care Provider has a greater role in coordinating your and your family’s
health care.
