HUSKY Primary Care - Connecticuts Primary Care Case Management Program
Information for healthcare providers
HUSKY Primary Care is a different type of Medicaid managed care. HUSKY A clients enroll directly with their Primary Care Providers (PCPs), instead of enrolling in a managed care health plan. PCPs are responsible for coordinating members’ health care, and receive a care coordination fee per member per month, in addition to fees for services provided. PCCM is used successfully in other states’ Medicaid Programs to improve patients’ clinical outcomes, and both client and provider satisfaction, while at the same time controlling health care costs.
The program is currently available in the greater Hartford, New Haven, Putnam, Waterbury, and Windham areas. We will soon be expanding the program to the Torrington areas. Please see below for more information.

follow this link. New Primary Care Providers in Hartford, New Haven, Putnam, Waterbury, and Windham, as well as the towns directly next to them, are welcome to join on an ongoing basis. DSS will soon be expanding the program to the Torrington area and we are currently seeking applications from Primary Care Providers in these areas.

follow this link. Medical services, providers, and facilities in the HUSKY Primary Care program are the same as in Medicaid/Title 19. Members use the gray CONNECT card for all services, and eligibility verification systems indicate the member’s Primary Care Provider.
