HUSKY Primary Care - Connecticuts Primary Care Case Management program


A new health care option called HUSKY Primary Care, Connecticut’s Primary Care Case Management program, is now available to HUSKY A members in the Hartford, New Haven, Putnam, Waterbury, and Windham areas.  We will soon be expanding the program to the Torrington areas.
With HUSKY Primary Care, you and your family members enroll directly with a participating Primary Care Provider, instead of enrolling first with a HUSKY managed health care plan and then selecting a Primary Care Provider.
In HUSKY Primary Care, your Primary Care Provider has a greater role in coordinating your and your family’s health care.  For example, here are some of the services you should expect from your Primary Care Provider:
• Linking you with community resources and services
• Helping you arrange transportation to appointments, if needed 
• Setting up appointments with needed specialists
• Providing same or next day appointments for urgent care
A major difference is between a managed care health plan and HUSKY Primary Care is this; your primary care provider is responsible for all of these aspects of your health care in HUSKY Primary Care, and in a HUSKY managed care health plan, your health plan is responsible along with your primary care provider. Under a managed care plan, your Primary Care Provider is also involved in a lot of the same things as in HUSKY Primary Care. The providers in HUSKY Primary Care offer the same services offered by a managed care health plan, such as health education, reminders about immunizations and well-child visits, and help scheduling appointments.
In HUSKY Primary Care, your gray Connect card is the only card you need to access your health care. Your Primary Care Provider will also offer some extra hours to contact his or her office, such as evening, weekend, or early morning.

All other benefits remain the same, as in any HUSKY managed health care plan. Mental health and substance abuse services, pharmacy and dental services and providers will remain the same, whether you choose to enroll in the HUSKY Primary Care program or a HUSKY managed health care plan.

When you are in the HUSKY Primary Care program, all of the healthcare providers you use must be enrolled with Medicaid/Title 19.  Your PCP will help you find and refer you to other providers. All of the hospitals in Connecticut are enrolled in Medicaid.

Each family member in HUSKY A will have a Primary Care Provider (PCP) who participates in the HUSKY Primary Care option.  Be sure to look at the list of available PCPs below or call the HUSKY program at 1-800-656-6684 to select a PCP. 
If you are in HUSKY A and live in the following towns, you can choose the HUSKY Primary Care option:
  • Hartford area: Hartford, East Hartford, West Hartford, Newington, Wethersfield, Bloomfield, Windsor, or South Windsor. 
  • New Haven area: New Haven, Hamden, North Haven, East Haven, West Haven, Orange, or Woodbridge.
  • Putnam area: Brooklyn, Killingly, Plainfield, Pomfret, Putnam, Thompson or Woodstock.
  • Waterbury area: Waterbury, Watertown, Plymouth, Wolcott, Cheshire, Prospect, Naugatuck, Middlebury or Thomaston.
  • Windham area: Windham/Willimantic, Chaplin, Scotland, Sprague, Franklin, Lebanon, Columbia, Coventry, or Mansfield.
HUSKY A members in other towns can also enroll if they or a family member are already a patient of a participating Primary Care Provider.
