Providers who wish to enroll as CHC Service Providers must be credentialed by Allied Community Resources (860-627-9500) and must fill out the enrollment application online at www.ctdssmap.com ---Provider---Enrollment. Please read the instructions carefully before starting the enrollment wizard. All required information must be submitted to complete the application.
