Primary Care sites assume the following obligations and responsibilities under the Connecticut SLRP program:

  • Primary Care Sites sponsor the application of health professionals to the SLRP.

  • SLRP contracts with the health professional are not to be used to offset salary. Salaries for health professionals participating in the SLRP should be based on prevailing rates in the area.

  • Priority for sponsorship will be given to clinicians who, after having completed their first two-year agreement, wish to sign a second two-year term.

  • Service credit will begin on the date an SLRP contract agreement is signed by the health professional, which may not be prior to the date that the SLRP participant begins full-time practice at the approved site.

  • SLRP participants must agree to accept Medicare, Medicaid, and State Children’s Health Insurance Program (SCHIP) assignment rates, and to treat patients regardless of their ability to pay (e.g. discounted sliding-fee schedule).

  • Payments shall be made directly by the health professional to the lending institution.

  • Health professionals shall be responsible for providing the Department of Public Health with documentation from the lending institution, demonstrating that each payment has been received and appropriately credited.

  • Payments to the lender must be made in a timely manner so as not to incur any late penalties, which will not be paid by the SLRP.
