Annual Consumer Satisfaction Survey

Monday, June 27, 2022:In recognition of the challenges that we have faced this year, particularly around staffing, we are extending the deadline for the annual FY22 DMHAS Consumer Satisfaction Survey.

The new Consumer Satisfaction Survey deadline will be Friday, July 15, 2022.  
All survey data MUST be in before 8 am on Monday, July 18 or it will not be included in our analysis.
