FY 2024 NSGP-NSS (Supplemental) Application - Due in the DEMHS Electronic Grants Management System by Monday, January 6th 2025.
To be considered for funding, a completed application must be submitted in the DEMHS Electronic Grants Management System, and each application will include the following:
- FY 2024 NSGP-NSS Investment Justification
- Within the DEMHS grants management system, prospective applicants will be able to download and save this form to a computer and access it using a PDF reader application, such as Adobe. The Investment Justification cannot be completed if the document is accessed in a web browser. The IJ form will be uploaded to the electronic grants management system at the time of submission.
- For additional guidance completing the Investment Justification, please review the NSGP-S Investment Justification Checklist from FEMA.
- FY 2024 NSGP-NSS Security Self-Assessment
- This is a required security self-assessment that must be completed before submitting in the DEMHS grants management system.
- The link to the self-assessment will be available in the DEMHS Electronic Grants Management System and the NSGP-NSS application.
- Please be as honest as possible when completing, to reflect current security infrastructure.
- Organization's Mission Statement - please upload a copy of your organization's mission statement on letterhead in the DEMHS Electronic Grants Management System
- Vulnerability Assessment - please upload within the DEMHS Electronic Grants Management System a written vulnerability assessment conducted within the past three years for each address applied for. This can be conducted by local law enforcement, a security contractor, or by completing the CISA Houses of Worship Self-Assessment.
NSGP-NSS FORMS AND REPORTING DOCUMENTS
- Environmental and Historic Preservation Form - required to be submitted by ALL awarded sub-applicants. Approval must be received before projects can start.
- Procurement Standards
- Conflict of Interest Form
- Quarterly Progress Report - must be submitted within 30 days of the close of the quarter (March 31; June 30; September 30; December 31)
- Quarterly Financial Report - must be submitted within 30 days of the close of the quarter (March 31; June 30; September 30; December 31)
- Reimbursement Request Form - must be submitted with copies of associated invoices and proofs of payments
- Equipment / Property Inventory Report - must be submitted with the final reimbursement request
