Required Forms

IRS Form 990 or IRS Form 990-EZ

Applicants are required to submit the organization's IRS Form 990 or IRS Form 990-EZ to substantiate total donor contributions.  Form 990-PF, Return of Private Foundation, and 990-N (e-Postcard) are not accepted.

The IRS form must be complete, signed and filed with the Internal Revenue Service (IRS) prior to upload/submission of application.

If the IRS form has not been filed, it cannot be used to qualify for the FY21 CT Arts Endowment Fund.

An organization's IRS Form for its Year 2 (Most Recent) must be for the organization's most recently completed fiscal year.

Not accepted are:

  • Draft IRS forms
  • IRS forms not filed with the Internal Revenue Service (IRS)
  • 990-N (E-postcards) and IRS Form 990-PF
  • Same two IRS forms submitted in FY20, if a returning grantee.

Certificate of Good Standing (aka Certificate of Legal Existence)

Applicants are required to provide a current* Certificate of Good Standing (aka Certificate of Legal Existence), from the Secretary of the State's Office. 

A current certificate is issued within one (1) year of the application deadline date of December 15, 2020.  A certificate issued prior to December 15, 2019 is not considered current. 

A Certificate of Good Standing can be obtained on-line by visiting the Secretary of the State's website HERE.  An Express certificate is acceptable.
