Congregate settings include individuals and staff in halfway homes, inpatient mental health facilities, corrections facilities, homeless shelters, domestic violence shelters, substance use and residential treatment facilities along with others. Vaccination programs for congregate settings are being coordinated by State and local health. These facilities are either licensed by or otherwise formally affiliated with the State of Connecticut and are administered by a private non-profit or other formal entity. In Phase 1b, congregate settings do not include supported apartments, foster or family settings, college dormitories, or boarding schools.
Residents and staff of facilities can make appointments to attend clinics at a range of community clinic locations – including through use of Vaccine Administration Management System, or VAMS, or directly through other scheduling tools used by providers.
The State, along with local health departments, and vaccine providers are working together to target solutions for residents of congregate settings who are adjudicated and need a guardian to sign a consent or who cannot get to a community clinic. Facilities can request assistance in accessing these solutions through this form.
If a request has been made through the form, matches will be made with a vaccinator in the next few weeks. Facilities using this process are being prioritized based on risk factors, so some facilities may be matched later than others. Vaccinators are currently scaling up capacity for these solutions, so please be patient.
