Requests for Mailing Lists
You can now generate free rosters on Health Care Professionals and Facilities licenses. The rosters can be downloaded in Text or Excel format which will enable you to easily format and sort the data. We hope that the information provided is helpful.

If you need additional information, that is not provided in the roster, please click here for information on filing a Freedom of Information Act (FOIA) request. Upon reviewing your request, our Communications Section will contact you with a response.
All requests will be processed in accordance to the Connecticut General Statutes, Sec. 1-212 .
