Material Change Notifications
 

Effective October 1, 2015, transactions that result in a material change to a group practice require the filing of a written notice to the Commissioner of Public Health not later than 30 days after the transaction effective date. The written notice shall identify each party to the transaction and describe the material change to the business or corporate structure of the group practice. Material changes are defined by 19a-486i(c).

 

What is the statutory authority for this new filing requirement?

Section 27 of Public Act 15-146 effective October 1, 2015.

 

When is this filing due?

Not later than thirty days after the effective date of any transaction.

 

Who needs to submit this new filing requirement?

The parties to the transaction shall submit written notice to the Commissioner of Public Health. The completed notification should be emailed to OHCA@ct.gov
