Petition for Clean Slate Erasure

Automatic erasure only applies to eligible convictions that were imposed on or after January 1, 2000.

To erase eligible convictions for offenses dated prior to January 1, 2000 under the Clean Slate program, you have to ask the court to erase your conviction.

1. Get the following documents to show to the court
  • A copy of your official criminal history record from the Connecticut State Police Criminal Records Unit
  • If you were released from prison on parole or special parole for any conviction and did not have probation as part of your sentence, you need a letter from the Department of Correction saying that you have completed your period of parole or special parole for every conviction.
    • You can request this letter from the Department of Correction Central Records Office by emailing DOC.CleanSlate@ct.gov. State in your email that you are asking for a letter of parole completion for Clean Slate and include the following information so DOC can find your records and contact you with any questions:
      • Full name
      • Date of Birth
      • Social Security Number
      • Inmate number
      • Phone number
      • Current mailing address
      • Email address (if you have one)
  • If you had probation as part of your sentence for any conviction, you need a letter from Adult Probation Services.
    • You can request this letter from Adult Probation Services by emailing CleanSlate.Requests@jud.ct.gov. State in your email that you are asking for a letter of probation completion for Clean Slate and include the following information so Adult Probation Services can find your records and contact you with any questions:
      • Full name
      • Date of Birth
      • Social Security Number
      • Phone number
      • Current mailing address
      • Email address (if you have one)

2. Complete the petition

Complete the Petition for Clean Slate Erasure, Convictions Before 1/1/2000, form JD-CR-202. You will have to swear under oath on the form that:

  • You have provided a DNA sample for any conviction that required it.
  • You are no longer required to register as a sex offender or deadly weapon offender for any conviction that required it.
  • You are not subject to any standing criminal protective orders.
  • You have paid the criminal fines for any conviction that imposed a fine as part of your sentence.

3. Attach the documents listed
Attach the documents listed above to the completed Petition for Clean Slate Erasure, Convictions Before 1/1/2000, form JD-CR-202.

4. File the completed Petition for Clean Slate Erasure

File form JD-CR-202 and the required documents with the court where you were sentenced for the conviction(s) you want the court to erase.

  • If you want the court to erase convictions in more than one docket number, you have to file a separate form for each docket number
  • If you were sentenced in one of the following courts that have since closed, file your petition at the court location listed below:
    • G.A. 6 at New Haven - file the petition at G.A. 23 at New Haven
    • G.A. 8 at New Haven - file the petition at G.A. 23 at New Haven
    • G.A. 13 at Enfield - file the petition at G.A. 14 at Hartford
    • G.A. 16 at West Hartford - file the petition at G.A. 14 at Hartford
    • G.A. 17 at Bristol - file the petition at G.A. 15 at New Britain
    • G.A. 20 at Norwalk - file the petition at G.A. 1 at Stamford

5. The court will review your petition

The court will either:

  • Order your conviction(s) to be erased without a hearing, or
  • Schedule a hearing on your petition to decide whether your conviction(s) should be erased
