                                   Grantee Reimbursement and Required Reporting 

Click the links below to open the necessary documents for your town.  Each town has two workbook links and one progress report link.  Progress reports are due quarterly.  The first progress report is due 20 days after the close of the first full quarter based on the date that the grant was fully executed (grant start date).

The workbook, with the name beginning “Reimbursements”, is the workbook which contains instructions and required forms for seeking reimbursement.  Please make sure to read the first worksheet on the tab labeled “READ THIS FIRST” as it provides important instructions.  Additionally, please make sure to read the instructions at the top of each form before completing them.

The second workbook, with the name beginning “Cumulative”, is the workbook that must be used to maintain a cumulative total of all of your requested reimbursement amounts by request and by category.  While you do not need to submit this workbook when requesting reimbursement, it still must be kept up-to-date as OPM may periodically request a copy.
