ARTE-accesible Grant Program

Final Report Deadline for Fiscal Year 2021 grantees: March 14, 2022

FY 2021 grantees must complete the final report using the SurveyMonkey Apply platform at https://ctofficeofthearts.smapply.org/

Log into your existing account to complete your final report. You will have access to your application as a reference. If you have forgotten your password select "forgot password" to reset a new one.

  1. Narrative: Provide a brief summary of the activity you carried out as part of this opportunity.
  2. Narrative: Explain the ways in which you feel your activity was a success and/or how your activity could have been more successful.
  3. Narrative: Explain any significant changes from the original application submitted and approved and indicate why these changes occurred (if applicable).
  4. Budget Upload: Document how COA grant funds were used. Grantees are encouraged to use our template Budget Form (for ARTE-accessible)
  5. Budget Narrative: Clarify any budget entries and/or changes to your original budget (submitted with application) that are greater than +/- 20% to the project's actual ending income and expenses.
  6. Credit & Publicity: Briefly share with us how was DECD/COA credited for its support of your activity?
  7. Supporting Documentation (optional): Provide link(s) to support materials documenting the activity.

Complete the National Standards for Arts Information Exchange Data Form*
The Connecticut Office of the Arts, as a recipient of federal funds from the National Endowment for the Arts (NEA), is required to report specific information to the NEA to document grant activities. Therefore, all grant recipients must complete the National Standards for Arts Information Exchange Data Form, and will be required to upload this document during the on-line process.  

* Grantees are required to complete the National Standards for Arts Information Exchange Data Form in its entirety.  Incomplete forms will be returned to the grantee for completion. 

Complete the Acceptance of Terms & Signature Form

Final Report Deadline for Fiscal Year 2020 grantees: January 30, 2021

FY 2020 grantees with an extension, you must complete the final report on-line using the traditional SurveyMonkey platform at https://www.surveymonkey.com/r/ARTE_Report. Please review the questions above to complete your report. You will also need to enter your Grant Number (found on page one of your grant contract).

Please note, when using the traditional SurveyMonkey platform you will not be able to save and return later. You must be prepared to complete your final report in its entirety once you begin. Please review the questions (below) and prepare your answers, and be sure to complete your budget and the National Standards Form (see below) before you begin the on-line final report.
