Connecticut Arts Endowment Fund (CAEF)

Final Report

The final report and its tasks are associated with/part of the Endowment application within SurveyMonkey Apply.

Instructions:

  1. Log in to your SurveyMonkey Apply account at: https://ctofficeofthearts.smapply.org

  2. Look for the “box” for your Endowment grant application and within that box look for the green “Start” button.

  3. Click the green “Start” button.This will open a page that will display “Your Tasks” for the final report.The Tasks are:
  • CAEF Final Report:Financials & Narrative
  • National Standard For Arts Information Exchange Data Form
  • Acceptance of Terms

5. To begin, click on the first task – CAEF Final Report:General Information.

6. Once you complete all tasks, be sure to submit your final report.
