Parental Consent

For students to participate in Project COVID DeteCT, parents and guardians must fill out the Project COVID DeteCT Parent Consent Form and return a signed copy to the school. Interested teachers and staff will need to complete a consent form to participate as well. The signed consent form will cover the duration of the weekly testing program. The Project COVID DeteCT program is funded through federal ELC resources.
