• Family Bridge Evaluation RFP 062923

  The Office of Health Strategy (OHS) is seeking proposals from public and private organizations to work with the OHS, the Office of Early Childhood (OEC), Bridgeport Hospital, and the Southwestern Area Health Education Center to create and implement an evaluation of the Universal Nurse Home Visiting- Community Health Worker (UNHV-CHW) Pilot Program of Greater Bridgeport known as Family Bridge.
