Child fatality review

We help keep children safe

The State of Connecticut reviews the deaths of children every year. The Child Fatality Review Panel (CFRP) performs these reviews. The CFRP is co-chaired by the child advocate. Its day-to-day operations are coordinated through the Office of the Child Advocate (OCA).

Each review looks at what went wrong and how agencies can keep children safe. Recent reviews have led to statewide changes in:

  • Child welfare practices
  • Mental health supports
  • Oversight for children in State care

Child Fatality Review Panel (CFRP)

Connecticut's Child Fatality Review Panel looks into unexpected deaths of children who got help from State services. These services include things like child welfare, social services, and juvenile justice.

The panel includes doctors, police, lawyers, and child safety experts. They work together to understand what happened and how to keep kids safe by:

  • Focusing on deaths that were unexpected or could have been prevented
  • Improving laws and safety programs across the state through findings with a focus on improvements
