I don’t have access to a smart phone or desktop application to submit my documentation. What should I do?

If you do not have access to a smart phone or desktop through work, family, public library or other trusted source, you may scan the necessary documents to WellSpark by fax at 860-678-5207 or 860-678-5229 or email at statecovid@wellsparkhealth.com. You will need to include a paper form of the signed attestation (see Forms under webpage) along with information that includes your name, address, phone, agency, and job title. If you need a way to print forms, please reach out to your human resources representative.
