How do I apply for an exemption?
There are two types of exemptions from COVID-19 vaccination that will be considered for those whose role requires it*:
1. Medical – for use when an individual’s physician (MD or DO), physician assistant (PA), or advanced practice registered nurse (APRN) determines that the administration of the COVID-19 vaccine is likely to be detrimental to the individual’s health. Click here for the form.
2. Religious or Spiritual – for use when an individual objects to the vaccination based on sincerely- held religious or spiritual beliefs and practices. Click here for the form
In both cases, the exemption needs to be validated and approved. If an individual is determined to be exempt, they still must submit to proof of a negative SARS-CoV-2 test result on a weekly basis. *Many roles, including those who work in health care facilities, require an exemption form to submit weekly test results rather than getting vaccinated. However, most state agencies do not require the above exemption forms and employees may chose to submit weekly test results.
