Record and Data Requests

Pursuant to Chapter 14 of the Connecticut General Statutes, anyone seeking public information from OHS can request it through the agency’s dedicated Freedom of Information website. Beginning August 14, 2023, please submit all requests for records and data using the agency’s new site available at the following link:

Please note that if the request seeks the disclosure of data that OHS collects, in particular Hospital Inpatient Discharge data, Emergency Department data, Ambulatory Surgery data, or All-Payer Claims Database (APCD) data, you will be required to complete and submit the appropriate request form set forth below. In addition, you may be required to pay a statutory fee prior to the disclosure of any such data.
