Application Based on an Out-of-Sate License

The Department may issue certification by endorsement to a doula who presents evidence satisfactory to the commissioner that the applicant is certified as a doula in another state or jurisdiction whose requirements for certification are substantially similar to those of this state.  The applicant must have been licensed for not less than two years before the date the doula submits an application for certification.

Applications are submitted online.  Please select this link to apply online.  After you have registered for an account and have verified your email address, please select "initial Application' link toward the bottom left corner of the screen, expand the 'Public Health Practitioners' grouping and then 'start' next to the Doula certification type.

Upon receipt of an application, the Department will attempt to verify that other-state certificates/licenses online.  The applicant will be notified in the event that the license(s) cannot be verified online.

Note:  This route to certification is only available to out-of-state certified/licensed doulas.  If the state where you are coming from does not issue certificates/licenses, please don't apply via this route.
