Using the App Religious Exemptions Medical Exemptions State Employees

No, all exemptions will need to be submitted to WellSpark via email at statecovid@wellsparkhealth.com or fax at 860-678-5207 or 860-678-5229. Please note that the medical exemption requires the signature and license number of physician (MD or DO), physician assistant, or advanced practice registered nurse.

Please note that the process referenced above is designed for state hospital employees only. Contractors can find further information about their requirements here.
