How do I prove I was tested?

Register on the COVID Navigator App on your smartphone or at CTState.medikeeper.com. The testing week runs from Monday to Sunday. Results need to be submitted via the app by midnight Sunday. Individuals required to test must receive at least one SARS-CoV-2 test every seven days to remain in compliance with the testing requirements.

If you do not have access to a smart phone or computer, you can submit your proof of testing via email at Statecovid@wellsparkhealth.com or fax to 860-678-5207 or 860-678-5229.

If submitting your test results via email or fax you must use the form located here and provide a copy of your test result.
