Providers who wish to enroll as CHC Service Providers must be credentialed by Allied Community Resources (860-627-9500) and must fill out the enrollment application online at www.ctdssmap.com ---Provider---Enrollment. Please read the instructions carefully before starting the enrollment wizard. All required information must be submitted to complete the application. Paper enrollment applications are no longer accepted. The provider manual can be accessed at www.ctdssmap.com ----Publications----Provider Manuals. Information about changes for providers can be found on the home page of www.ctdssmap.com under Publications, Provider Bulletins and Important Messages. Provider may also access free provider billing training sessions conducted by HP.
